Teeth Clenching and Grinding (Bruxism)

Teeth clenching and grinding are bruxism behaviours involving involuntary contact or movement of the teeth. They can occur while awake or during sleep. Some people have no symptoms, while others develop tooth wear, fractures, tired jaw muscles or morning pain.

Bruxism should not be attributed to a single cause. Sleep, stress, medication, stimulants, associated sleep disorders and individual factors may all need consideration.

What Is Bruxism?

Awake Bruxism

This involves clenching the teeth or holding the jaw muscles tense during the day. It may be associated with concentration, stress or habit, and can occur without conscious awareness.

Sleep Bruxism

This is rhythmic or non-rhythmic jaw-muscle activity during sleep. It is considered a sleep-related movement behaviour rather than a simple habit.

  • Flattened or worn tooth surfaces
  • Cracks, fractures or sensitivity
  • Repeated damage to fillings, crowns or veneers
  • Tired or painful jaw muscles on waking
  • Temple headaches

These signs are not specific to bruxism. Dental, muscular, joint and other possible causes should be considered.

What Causes Bruxism?

  • Jaw tension during stress, anxiety or concentration

  • Snoring or sleep apnoea
  • Selected antidepressant or stimulant medication
  • High caffeine intake, smoking or alcohol
  • Some neurological or developmental conditions
  • A family history of bruxism

The way teeth meet should not be assumed to be the sole cause. Irreversible adjustment of healthy teeth is not a routine first-line treatment for bruxism.

How Is Bruxism Assessed?

The assessment reviews tooth wear, cracks, restorations, muscle tenderness, jaw movement and joint findings. The timing of symptoms, sleep quality, medication, caffeine and stress are discussed.

Significant snoring, witnessed pauses in breathing, daytime sleepiness or morning headaches may require sleep-medicine assessment. A sleep study is not necessary for every patient.

What Does a Night Guard Do?

A custom occlusal splint can separate the teeth and help reduce wear and restoration damage. It may support the management of muscle and joint loading in selected patients.

A guard does not remove every cause of bruxism or guarantee that grinding will stop. Appropriate design, review and replacement when worn are important. Unsupervised over-the-counter appliances can adversely affect the bite or symptoms.

How Is Bruxism Managed?

  • Awareness of daytime jaw posture
  • A custom night guard
  • Review of sleep and lifestyle habits
  • Assessment of caffeine, nicotine and alcohol intake
  • Exercise or physiotherapy referral for muscular symptoms
  • Medication review with the prescribing clinician
  • Referral when a sleep disorder is suspected
  • Restoration of damaged teeth when required

Management is individualised. There is no single treatment that is appropriate for every patient.

How Can Awake Clenching Be Recognised?

At rest, the lips may touch lightly but the teeth do not need to remain in contact. Phone reminders or visual prompts can increase awareness. When tension is noticed, relaxing the shoulders, resting the tongue and separating the teeth may help.

How Can Bruxism Damage Teeth?

Prolonged or high-force activity can contribute to enamel wear, cracks, sensitivity and restoration failure. Bruxism risk should be considered before rebuilding worn teeth so that new restorations are not exposed to unmanaged forces.

Is Jaw-Joint Pain Always Caused by Bruxism?

No. Jaw-joint and muscle pain can relate to trauma, internal joint changes, muscular overload, head and neck pain or other conditions. Clicking alone may not require treatment, but pain, locking or restricted movement should be assessed.

Does Bruxism Affect Orthodontic Treatment?

Bruxism is not an automatic barrier to orthodontics. It may influence appliance breakage, muscular symptoms and retention planning. The choice between aligners and fixed braces is not based on bruxism alone.

Is Botulinum Toxin Used for Bruxism?

Botulinum toxin injections into jaw muscles may be considered in selected cases for muscle activity or pain management. They do not treat every cause of bruxism, do not physically protect the teeth and have a temporary effect. Suitability, adverse effects and alternatives require assessment by an appropriately qualified healthcare professional.

Grinding in Children

Sleep grinding can occur in children and does not always require treatment. Tooth wear, pain, sleep disturbance, snoring or symptoms affecting daily life should be assessed by pediatric dental and, when required, other healthcare professionals.
